Executive Summary

Operational Context:

• South Sudan is in a compound acute crisis: 7.8 million people (56% of population) in IPC Phase 3+ for April–July 2026, including 73,000 in IPC Phase 5 (Catastrophe) across Akobo, Fangak, Nyirol, and Uror (Jonglei) and Luakpiny/Nasir and Ulang (Upper Nile) — a 160% increase in the Catastrophe caseload versus the prior IPC cycle. Famine risk is confirmed under worst-case scenarios in all four named counties, driven by active conflict, humanitarian access denial, and livelihood collapse.^[1]
• Security deteriorated sharply in Q1 2026: UNMISS documented 1,388 civilian casualties, with civilian deaths up 89% quarter-on-quarter despite fewer incidents — signalling higher-lethality violence.^[2] ACLED records 91 conflict events and 171 fatalities in June 2026, down 27% and 34% respectively from May, but well below the 12-month average of 145 events/404 fatalities/month, suggesting a temporary lull rather than structural improvement.
• A fatal ambush in Duk County, Jonglei on 29 June 2026 killed 17 people including five staff from the John Dau Foundation, a WFP local partner, returning from a WFP-affiliated training — a direct operational security shock in a critical access corridor.^[3]
• Insecurity in North and West Kordofan (Sudan) — including RSF attacks on El Obeid, a key logistics junction for cross-border supply routes into South Sudan — threatens pipeline continuity. The approaching rainy season will further degrade road access across Jonglei and Upper Nile.^[4]
• EVD cross-border transmission risk is elevated: the DRC–Uganda–South Sudan corridor has identified South Sudan entry points via Nimule, Rei River, and Olikwi; no confirmed South Sudan cases exist, but cross-border population movement is characterised as ongoing and porous.^[5]

Vulnerability Scale:

• 2.2 million children under five are at risk of acute malnutrition; 46% of 102,105 cumulative cholera cases are children aged 0–14, with 55% of new cases in WHO Week 25 (181 of 329) among IDPs — 174 in Rubkona and 7 in Awerial (Unity State).^[6] Unity accounts for 33% of cumulative cholera cases; cholera CFR exceeds the emergency threshold.
• Approximately 30,000 IDPs in Lankien, Nyirol County are surviving on wild leaves and fishing; the Lankien market is non-functional with no cereals available — a near-famine coping profile consistent with IPC Phase 4+ for this sub-population.^[7]
• Over 304,000 people have been uprooted in Jonglei since December 2025; approximately 36,135 asset-depleted returnees from Ethiopia's Gambella Region arrived in Akobo and Uror counties between March–April 2026.^[1]
• National food basket cost stood at SSP 120,734/person/month (week of 8–14 June 2026), up 6% month-on-month, with Upper Nile recording +11% driven by sorghum prices in Renk. A simultaneous multi-commodity supply shock in Rumbek (Lakes State) saw salt +180%, wheat flour +60%, sukuma wiki/goat +100% — signalling localised market collapse with implications for CBT effectiveness.^[8]
• Severe rainfall deficit affects 18 of 78 districts across Western Equatoria, Lakes, and Eastern Equatoria (worst: Panyijiar at 66% of seasonal normal); acute 10-day shortfall in 43 districts (worst: Kapoeta North at 17%). Vegetation indicators have not yet reflected the deficit — watch-level, but a deterioration signal for the agricultural season.

Trend Indicators:

• The April 2026 IPC analysis represents a significant deterioration from September 2025 (Phase 3+: 7.8M vs. 5.97M then), with the Catastrophe caseload up 160%. The September 2025 IPC is now 10 months old; the April 2026 FSC projection is the authoritative current basis.^[1]
• Food prices are broadly rising: WFP VAM data (May 2026) shows wheat flour +14% m/m, maize (white) +52% m/m, sorghum (imported) +21% m/m, and livestock prices up 11–25% m/m — consistent with lean season onset and supply constraints, and corroborating the food basket cost trajectory.
• Cholera trajectory: 102,105 cumulative cases as of 10 April 2026; ICG has approved 435,477 OCV doses for mop-up in Rubkona, Mayom, and Leer, but operational funding for campaign logistics remains under mobilisation.^[6]

WFP Operations:

• WFP reached 494,000 people in April 2026 (489,300 directly by WFP), against a monthly target of 2.1 million — a 24% coverage rate and 76% gap. WFP is effectively the sole delivery agent under Cluster Objective 1; IRC, the next largest partner, reached only 3,500 people.^[9]
• Critical suspension: WFP has delivered zero food assistance in Ulang County throughout all of 2026 following the looting of WFP commodities in Baliet and suspension of river transport. A planned airdrop resumption from July 2026 is subject to security and feasibility confirmation — currently unconfirmed.^[7]
• Named coverage gaps: Akobo East — 28,596 of 76,950 verified beneficiaries unreached (37% gap); Akobo West July–August distributions not yet commenced due to insecurity; Dengjok (target 8,165), Gakdong (3,807), and Kaikuiny (3,878) payams have zero coverage against target.^[10]
• Nasir: 92,654 beneficiaries received food assistance (Jan–May 2026) but zero livelihood assistance — an explicitly flagged gap.^[7] Uror: 132,492 people assisted with 2,325 MT (March–June 2026); July–August distributions ongoing.
• WFP-supported Concern Worldwide AAP projects are operational in Renk, Aweil, and Malakal (May 2026), providing community feedback infrastructure via Radio Community and Community Engagement Networks that can be leveraged for cholera and EVD risk communication.^[11]
• Previously (2024 ACR): SO1 exceeded beneficiary targets (126% beneficiary achievement rate) due to unplanned flood and Sudan crisis response, funded at 98% of revised plan. SO2 reached 84% beneficiary achievement rate; SO3 reached 57% beneficiary achievement rate, with SO3 resilience activities funded at only 42%. SO4 reached 78% beneficiary achievement rate at 62% funding rate. Under the 2023–2026 CSP, WFP planned USD 3.77B total and 7.2M beneficiaries across the plan window — planned figures only, not current reach.

Funding Outlook:

• As of May 2026, only USD 25M received against a USD 410M April–July 2026 requirement — approximately 6% funded, leaving a USD 385M+ gap. Current cluster delivery is at 33% of planned scale (900,000 cumulative vs. 2.7M target).^[1]
• A CERF allocation of USD 6M has been announced for the May–July readiness and response plan (estimated total: USD 7.3M), leaving a USD 1.3M gap for that specific envelope.^[12]
• Global donor competition is intensifying: a 43% surge in global cholera cases in May 2026 (16 countries), the BVD multi-country EVD appeal at USD 115M with 71% unfunded, and Sudan emergency RRRP requirements are competing for the same donor pool simultaneously.^[5]
• Collapse of USAID programming across WFP's key operational theatres — including South Sudan — represents a sustained structural shock to the humanitarian architecture.^[13] Previously (2024 ACR): WFP revised its annual budget from USD 1.2B to USD 897M (27% reduction), ultimately mobilising USD 1.08B including carry-over; 19.9% CSP funding gap recorded.
